Nuvance Health is seeking a 32 hr. Part Time and 40 hr. Full Time LCSW for our primary care office based in Norwalk, CT. We offer a highly competitive compensation package including base salary plus a $5,000 sign on bonus for full time and pro-rated for part-time.

Summary:

Provide behavioral health consultation to children, adolescents, adults and families with behavioral health needs with the goal of improving psychosocial functioning of Health Quest Primary Care patients. Performs assessments and formulates treatment plan and or discharge/transition plans in collaboration with patient, provider, and possible family and multidisciplinary healthcare team. Provides ongoing monitoring and evaluation of plans to meet the individual needs of patients throughout the care continuum. Assists in resource management through facilitation of care and participation in program planning, data collection and analysis. Provides education to patients/families regarding coverage, care options and community resources.

Qualifications:

  • Master's degree in Social Work from an accredited school

  • aMinimum of 3 years’ experience

  • Current CT Licensed Clinical Social Worker (LCSW) in CT

Norwalk Hospital has 366 beds and is also a Level II Trauma Center with approximately 50,000 ED visits per year.
